The first is PHL 2165, which requires proof of immunization or the absence of susceptibility to Measles, Mumps, and Rubella (German Measles). This PHL applies to all students born on or after January 1, 1957 who are taking 6 or more credit hours. Students born before January 1, 1957 do not need to prove susceptibility. The second, PHL 2167, states that all students, regardless of age, must prove they have received information about their risk of meningitis and that they are aware there is a vaccine that could reduce this risk. Evidence of a meningitis vaccine within the last 10 years or a signed waiver declining the vaccination satisfies this PHL.
All students, regardless of credit hours, are encouraged to submit a Pre-enrollment Health Form to avoid registration conflicts. A pre-enrollment health form can be downloaded here, or obtained from the Health Office, Advising and Counseling Office, and the Welcome Desk on the Elmira campus.
Proof of Immunity to Measles, Mumps, and Rubella can be satisfied by providing a copy of your vaccination record and should include evidence of:
- Two doses of a LIVE Measles vaccine given no more than 4 days prior to the student's first birthday, and the second dose given a minimum of 30 days from the first dose.
- One dose of a LIVE Mumps vaccine given no more than 4 days prior to the student's first birthday.
- One dose of a LIVE Rubella vaccine given no more than 4 days prior to the student's first birthday.
Most students born after January 1972 have received two MMR's which is the equivalent of receiving:
- Two doses of Measles
- Two doses of Mumps
- Two doses of Rubella
Record of two MMR's exceeds the New York State requirement. In most cases these two vaccines are completed prior to Kindergarten. If you are unsure about your vaccine history see obtaining records for CCC.
Religious and Medical Exemptions
Persons who hold genuine and sincere religious beliefs contrary to immunizations will be exempt from PHL 2165 after submitting a completed request that has been notarized and approved by the Vice President and Dean of Student Development. Religious exemption requests should be submitted to the Health Office for consideration using the following mandatory form.
An additional health form which includes a physical and additional vaccines is required for Athletes, Nursing students and Police Academy Cadets. Contact the Health Office or the following departments to request a physical form and deadlines for submitting:

All students that remain non-compliant following a 30-day grace period, risk being removed from the campus until compliance can be proven. HOLDS will be placed on all student accounts following the end of the grace period that have not provided the Health Office with a pre-enrollment health form, adequate vaccine history, and evidence of meningitis education. HOLDS prevent students from registration, schedule changes, transcript request, obtaining grades, and could result in delays with graduation. In the event of a campus outbreak, all susceptible students will be removed from campus until public health officials declare it is safe to return to campus or until proof of vaccination history has been determined.
