2013-2014 (Fall 2013, Spring 2014 & Summer 2014) forms below.

The forms below are for students who received a letter from Financial Aid asking them to complete one or more of the following forms for academic year 2013-2014.

Verification Forms:

2013-2014 Asset Information Page  (ONLY IF REQUESTED)
Needed if you filed a 1040 and were not able to file a 1040A or 1040EZ and your asset information was left blank.

2013-2014 Dependent Student Low Income Questionnaire  (ONLY IF REQUESTED)
Needed if your FAFSA was filed with no source of income reported for parents and/or none reported on your verification worksheet as well.

2013-2014 Dependent Student Verification Worksheets  (ONLY IF REQUESTED)
Needed if your FAFSA was selected for verification through the Pell Grant Processing Center. You are required to complete one or more of these forms and return to the Financial Aid Office before your financial aid can be determined.

2013-2014 Inability to Obtain Parental Information Form  (ONLY IF REQUESTED)
Needed if you filed your FAFSA stating you cannot provide parental information because of parent's unwillingness. The form is required to be notarized.

2013-2014 Independent Status Form  (ONLY IF REQUESTED)
Needed if you indicated on your FAFSA that you were independent for reasons other than being 24 years old, married, have children of your own or are a veteran. Your reason needs to be verified with the appropriate documentation. Also used if you cannot prove your reason for independency and wish to withdraw your independent status and have your financial aid processed as a dependent student.

2013-2014 Parent Information Form  (ONLY IF REQUESTED)
Needed if your FAFSA did not include parental information when it was required.

2013-2014 Proof of Separate Residences  (ONLY IF REQUESTED)
Needed if you have separated or divorced from your spouse (if an independent student) or your parents have separated or divorced (if a dependent student) after your FAFSA was filed.

2013-2014 Signature Page  (ONLY IF REQUESTED)
Needed if your FAFSA was filed without any or all required signatures.

2013-2014 Verification of Dependent Support  (ONLY IF REQUESTED)
Needed if your FAFSA was completed indicating you have a legal dependent(s) other than a spouse and you reported low or no income. A copy of the dependent(s) birth certificates are required as well.

Loan Forms:

2013-2014 Direct Loan Increase/Decrease Request Form - If you would like to have your originally accepted loan amounts to be increased or decreased.

2013-2014 Parent PLUS Loan Request Form - If your parents plan to borrow a loan on your behalf based on being a dependent student. A Master Promissory Note is required to be completed by one parent as well at www.studentloans.gov.

2013-2014 PLUS Loan Denial Form - If your parent's were denied a PLUS loan and you wish to borrow additional loan in your name above the standard maximum amounts based on your grade level.

Miscellaneous Forms:

2013 Academic Progress/Federal Aid Appeal - If you did not meet academic progress per the Federal guidelines and are required to appeal to have your federal aid reinstated for Summer/Fall 2013 use this form (due October 31, 2013). This form can also be used for Academic Standards appeals to be submitted to the Advising and Counseling Office (due July 17, 2013).

2013-2014 APTS (Aid for Part-time Study) Application - Application will be available June 2013. If you are a New York State resident attending on a part time basis (1-11 credit hours per semester). 

2013-2014 Dependency Appeal Form - If you do not meet the standard criteria to be an independent student and you have extenuating circumstances to be considered to be an independent student. Extensive documentation is required for proof.

2013-2014 Dependent Student Special Condition Form - If your parent's household situation has changed after your FAFSA was filed based on loss of employment, separation, divorce or decrease or loss of untaxed income to reflect a lower income level for the current academic year compared to the tax year information that is required.

NYS TAP One-time Waiver - New York State regulations permit students to receive a one-time waiver of the good academic standing requirement as an undergraduate. Specific details regarding the extenuating circumstance(s) need to be reported, such as events and dates, and appropriate third party documentation is required whenever possible.

Waiver of C-Average Requirement - The C-average requirement may be waived for undue hardship based on the student's personal illness or injury or other extenuating circumstance(s). Specific details regarding the extenuating circumstance(s) need to be reported, such as events and dates, and appropriate third party documentation is required whenever possible.

2013-2014 Financial Aid Budget Increase Request for Purchase of Automotive Tools - If you are requesting that the CCC Financial Aid Office increase your financial aid budget for the purchase of required tools for the automotive program.

2013-2014 Housing Adjustment Form - If your housing status has changed and/or was reported incorrectly on your Free Application for Federal Student Aid (FAFSA).

2013-2014 Federal Student Financial Aid Penalties for Drug Law Violations - A federal or state drug conviction can disqualify you for FSA funds.

Convictions only count if they were for an offense that occurred during a period of enrollment for which you were receiving Title IV aid - they do not count if the offense was not during such a period. Also, a conviction that was reversed, set aside, or removed from your record does not count, nor does one received when you were a juvenile, unless you were tried as an adult.

The chart below illustrates the period of ineligibility for FSA funds, depending on whether the conviction was for sale or possession and whether you had previous offenses. (A conviction for sale of drugs includes convictions for conspiring to sell drugs.)

If you answered "Yes" to question 23 on the FAFSA which is "Have you been convicted for the possession or sale of illegal drugs for an offense that occurred while you were receiving federal student aid?", you must complete the attached worksheet.
